Instructions: Provide all information. We suggest you make a copy of this form for your records. You can download a copy, print it from your browser, fill it out, sign it and return it to:

If your browser does not support applications, phone or send us an email at info@mlsa.com for an application. All applications submitted require $100 application processing fee, payable to MLSA and five recent passport size photos (2" X 2") with your name on the back.

It is recommended to send the applications as early as possible because some programs fill up fast and some of the courses you want to take may be closed, if you wait too long.

DEADLINE FOR APPLICATIONS
Applications must be received by:
October 30 winter program
April 10 summer II program
Applications for all other programs are due 95 days before beginning of the program.
(If sent after deadline, please include a $100 late-applicant fee.)

MLSA will confirm in writing your acceptance into the program within two weeks after we receive your application.

The form must be signed by the parent or guardian and public notary, if the student is under 18 years of age.
