APPLICATION REQUIREMENTS
Initial applications will only be accepted and considered completed when they are submitted with the following materials:
- Application form completed with all information requested.
- Release & Agreement form legibly printed and signed.
- Be sure to include full name as it should appear in all the academic documentation AND two current email addresses (sometimes schools block our emails due to filters, AND EMAIL IS OUR MEANS OF COMMUNICATING ALL INFORMATION).
- Enclose/Email a digital passport-size recent HEAD SHOT photo (minimum "fine" quality, portrait) to info@mlsa.com.
- $100 registration fee (non-refundable).

For groups, deadlines for applications and payments may vary—please check with your sponsor.
If sent after deadline, please include a $200 late applicant fee.
